All,
Never heard of a "doll" light when discussing N&W signals (on the Clinchfield, they were blue). I have seen N&W masts for a track intervening (Shenandoah Junction comes to mind).
I do have a semaphore like the one pictured (some survived as train order signals) hanging in my den. The casting was also used as a counterweight on crossing gates.

appears to show electric "marker" lights below the semaphore arms as well as the doll posts. Would these have been used to make the dolls visible at night? Would they have shown red? How about the one below the two-arm semaphore? Would this have been used to indicate stop and stay at some point?
