Boiler Explosion, Y5 2114, Cincinnati District

This explosion was covered in ICC Report No. 3353 and probably it does not
appear in DOT's investigations because the report was made by the Locomotive
Inspection Bureau, rather than the Safety Division Usually, mechanical
failures were investigated under the Locomotive Inspection Act. The report
indicates that "the accident was caused by overheating of the crown sheet
due to low water." Daily inspection reports submitted by engineers after
each
run indicate that the 2114 had been cited for water pump and injector
problems
eight times beginning on August 1, 1950.

One spectator noted "that the front of the boiler ascended above the tree
tops and
steam was discharging from the front of the boiler; that flying parts were
visible
moving toward him."

Another spectator noted "that the boiler was visible above the tree tops;
that
the back end of the boiler descended and made a hole about 6 feet in
diameter and then jumped."

Louis Newton once noted that a telephone pole near the explosion had been
pierced by one of the flying parts and it kinda stayed lodged in the pole
for a
long time after the incident.
