Turnout derailment

Since the trailing truck is derailing, perhaps examine the track beyond the turnout. The Y is a long locomotive so may be a radius issue. Does it derail no matter the direction or orientation of the locomotive?If it’s not heretical to suggest, maybe try a 6 axle or 4 axle diesel to see what happens. Where in the turnout does it derail?
