Don't know about the general case, but it may be true of any steam loco  with 
four cylinders and two sets of drivers.  One consultant on the design  of 
PRR's T-1 (4-4-4-4) suggested having different driver diameters for each set  to 
minimize the possibility.  Maybe it's more likely to occur with larger  driver 
diameter ?  Or lower factor of adhesion ?
